Juntamente com Metallica, Megadeth e Slayer o Anthrax é considerado uma das bandas responsáveis pelo surgimento e definição da sonoridade que viria a ser conhecida como thrash metal, variante mais rápida e enérgica do heavy metal tradicional. O primeiro disco da banda, "Fistfull of Metal", é considerado um marco do estilo. Apesar da comum associação com o thrash e speed metal, o Anthrax (o nome foi tomado de um microorganismo desenvolvido para guerra bacteriológica) dificilmente poderia ser rotulada visto que flertou insistentemente no decorrer da carreira com estilos musicais tão diversos como metal, punk, hardcore, hip-hop, rap e industrial. As letras, ao contrário da temática clássica de bandas semelhantes, são bem humoradas e imprevisíveis.
